I recently launched a side project called Feedbask with a dev friend. It’s a widget you can embed on your site to collect some or all of these :

• Bug reports
• Feature requests
• Testimonials
We built it because we felt existing tools were either too complex, incomplete or too expensive. It's simple, you just copy a script, customize a few things, and it's good to go.
It’s still early, so I’d love to hear your feedback – good or bad. Especially if you’ve built a product and face the need to get user feedbacks.
